Application Procedures for the Don H. Harris Activity Awards


 •  The deadline for turning in entries for our Activity Awards is on or before April 13Entries may left in the CCMTA crate at The Music Rack, or mailed to Don Harris beforehand at 3218 Forest Glen, Spring, TX 77380.

 •  Please use the application form found on page 56 of the yearbook to list accumulated points earned from May l, 2003 to April 30, 2004.   A separate form must be filled out for each individual student applying for the award.  (This form is also available on our website at ccmta.org under Activity Entry Forms—where each individual form may be filled out on computer and then printed out.)  Note:  Grade level refers to the school grade at time awards are presented, even though points actually start accumulating from May of preceding school year.

 •  We will be confirming points for all Student Affiliate events through the participation records of our SA chairmen; therefore, no extra documentation (copies of theory tests, critique sheets, etc.) will be required.

 •  We will, however, need documentation for the last three categories related to “non-SA performances” (Solo Recital, Studio Recital, and Other Performances).  If no programs or other printed forms of documentation were available for a specific performance in this category, a brief note describing nature of the event and signature from the teacher will be considered acceptable.  (When several entering students from the same class performed on the same program, turn in only one copy of the program with the appropriate names highlighted.)

 •  Please read pages 55 -56 in the yearbook thoroughly before sending in entries.  The information contained on these pages should clarify all the present specifics of the awards.  However, if there should be any further questions, feel free to call or e-mail Don Harris at your earliest convenience.  (281-367-4473 or donhharris@charter.net)

 •  Teachers of Cash Award Winners will be notified by phone and/or email shortly after winners are determined.  In addition, a complete breakdown of winners will appear in the May newsletter.

 •  The Awards Ceremony and program of original compositions will take place at 3:00 PM on May 2 (Sun.) at H&H Music.  At that time, cash awards totaling $500.00 plus achievement awards for all entering students will be presented.  All involved students, parents and teachers are strongly encouraged to join in enthusiastic support of this exciting new CCMTA opportunity by being present on May 2.
